To Receive a dental quote, please fill out the Census Form and check off Blue Cross Dental or Delta Dental (if you have 3 or more employees interested in Delta Dental).

Groups with 1 or 2 employees: Entrepreneur Platinum Option 2 plan is available.

Groups with 3 or more employees: Several plan options are available, please call our office for a quote.

Groups with 3 or more employees: Delta Dental plans are also available, please call our office.

You will be notified of your final rates by mail. If you decide to enroll in Blue Cross, the following must be returned to GNAC no later than the 25th of the month, for coverage to be effective the 1st of the following month:

One month's premium for medical and dental (if applicable) payable to:
