Need the Following Tax Verification
when Applying for Health Insurance
:

* State Quarterly Tax & Wage Report required for all groups to verify employment status. Each employee must be listed, along with wage information.

* For a "C" Corporation - Form 1120s, and Payroll

* For an "S" Corporation - Form 1120S, and Payroll

* For a Partnership - All K1's and Payroll

* For a Sole Proprietorship - Form 1040/Schedule C, and Payroll (if applicable)

* For a Church - Form 941 and Payroll

* For a Limited Liability Corporation (LLC) - LLC Agreement and if co files as a C Corporation or a Partnership, see the above for appropriate documentation.

* If your business has been in existence less than one year and has not filed - Please complete the Affidavit for Employer/employee not on payroll
